Documentation to bring with you

Please bring the following documents to your first appointment:

  • Referral slip from the neurologist, family doctor or internist or other doctor,
  • Chip card

These formalities are not required for private patients and international patients.
For all patients regardless of their insurance status, the following documents are helpful and you should bring them with you:

  • Doctor’s letters on diagnosis and treatment in other clinics
  • Results of investigations (electromyography, electroneurography, CSF analysis etc.)
  • Films of any MRI and CT scans that have been performed

Please bring a referral slip and the chip card to subsequent visits.

Before the doctor patient discussion, the following information, which you can prepare at home, will be recorded by the study nurses. We recommend assembling the information before your appointment and if necessary noting it down on a separate piece of paper. In the first part of your outpatient clinic appointment, you will be asked for the following information at a specially provided computer and in conversation with outpatient clinic staff:

  • Name, address and telephone number of your family doctor
  • Name, address and telephone number of your neurologist
  • Name, address and director of the clinic making the diagnosis
  • Name and address of your health insurance and your insurance number
  • Relevant previous diseases with year of onset
  • Current medications with their trade names, dose and time the medications are taken

In the first part of the doctor patient discussion, you will be asked about your medical history. You can prepare partially for the specific questions by recalling the time course of your symptoms while you are still at home. You can expect the following questions:

  • Have you any weakness of the arms? If so, since when? (state month and year)
  • Have you any weakness of the legs? If so, since when? (state month and year)
  • Have you noticed a change in your speech? If so, since when? (state month and year)
  • Do you feel any impairment of your breathing? If so, since when? (state month and year)

The preparation for the outpatient appointment is used to reduce the administrative part of the visit and thus leave more time for consultation and discussion of your individual diagnosis and treatment options. We also understand that visiting our outpatient department is not an everyday situation for you and can be associated with some agitation. In this situation, your documents can be a “memory prop“.
